Side sleeping and fibromyalgia have an awkward relationship. It is the position most people find natural and the one clinicians rarely argue with, but it concentrates body weight through the hip and shoulder, and in fibromyalgia those are precisely where tender points cluster. The result is familiar: comfortable at lights-out, aching by 2am, turning constantly by 4. The fix is not abandoning side sleeping. It is choosing a surface that stops the position loading your most sensitive points, and setting the rest of the body up so it does not compensate.

What the surface has to do

A side sleeper's hip and shoulder carry most of the load, and with fibromyalgia, sustained pressure on a tender point is not background discomfort, it is a signal a sensitised nervous system amplifies until it wakes you. The mattress needs conforming comfort layers deep enough for hip and shoulder to settle in and spread their load across a wide area, so no single point carries it. Underneath, the support layer must keep the pelvis level: sink too far and the spine bends sideways all night, trading pressure pain for a sore back and waist. Soft where you touch it, supportive beneath, the hybrid formula, is built for exactly this. And responsiveness matters more than most reviews admit: fibro side sleepers reposition often, and on a slow, deep foam every turn is effortful enough to wake you fully, while pocket springs assist the movement and let a half-asleep shuffle stay half-asleep.

“Sustained pressure on a tender point is not background discomfort. It is a signal a sensitised nervous system amplifies until it wakes you.”

The rest of the side-sleeping set-up

A pillow between the knees keeps the hips stacked rather than rotated, sparing the lower back and the hip you are lying on. The head pillow should bridge the shoulder gap fully, holding the neck level, a flattened pillow lets the head drop and adds neck and shoulder tenderness to the list by morning. Smooth, soft bedding and seam-free nightwear remove the small sensory irritations a fibro nervous system notices all night. And alternate sides when you can: giving each hip and shoulder nights off spreads the load across the week, not just the mattress.

Frequently asked questions

Should I switch to back sleeping instead?

Only if side sleeping stays painful on a properly supportive, pressure-relieving surface. Back sleeping spreads weight more evenly and suits some people with fibromyalgia well, but many simply cannot maintain it, and a well-set-up side position beats a back position you fight all night.

Why do my hips only hurt in bed?

Side lying loads the hip in a way standing never does, sustained sideways pressure through the joint and the soft tissue over it, and fibromyalgia amplifies how that pressure registers. A more conforming surface usually transforms it, but hip pain that persists despite a good set-up is worth a GP or physiotherapist review, as bursitis commonly coexists and is treatable in its own right.

Soft mattress or medium-firm for fibromyalgia side sleeping?

Medium-firm with genuinely soft comfort layers. An all-soft mattress feels wonderful for twenty minutes, then lets the hips sink out of line for eight hours and hands you a back problem to go with the fibro. Softness belongs at the surface, support belongs underneath.
