Shoulder arthritis has a particular cruelty for side sleepers: the position most of us find natural puts your full upper-body weight directly through the painful joint. Fixing it is a two-part job, the mattress decides how much load the shoulder carries, and the pillow decides what angle the neck and shoulder spend the night at. The mattress half: unloading the shoulder

When you lie on your side, the shoulder is one of two points carrying most of your weight. On a surface that is too firm, it carries that load concentrated, and an arthritic shoulder will wake you to complain about it, often more than once. The fix is comfort layers deep enough to let the shoulder sink slightly and spread its load, over a pocket spring core that keeps the spine level while it does. Too soft is its own failure: if the whole torso sinks, the spine bends and the neck follows. Medium-firm with genuine cushioning at the surface is the reliable answer for shoulder pain, which is why an outright orthopaedic-firm bed, so often assumed to be the arthritis choice, frequently makes shoulder arthritis worse.

The pillow half: setting the angle

Side sleeping opens a gap between the mattress and your head equal to the width of your shoulder. A pillow too low lets the head drop, kinking the neck and dragging the shoulder joint with it; too high tilts everything the other way. The job is a pillow full enough to bridge that gap and firm enough to hold its shape doing it, all night, every night, which a flattened hollowfibre pillow simply cannot do. Sleeping on the unaffected side where possible, with the painful arm supported rather than trapped beneath you, completes the set-up, and a small pillow hugged to the chest gives the top arm somewhere to rest that does not twist the sore shoulder.

Should I stop sleeping on my side altogether?

Not necessarily. Sleeping on the unaffected side with proper mattress and pillow support works well for many people. If both shoulders are involved, back sleeping on a supportive surface with a suitable pillow becomes the position worth cultivating, changes of habit take a couple of weeks but do stick.

Why does my shoulder hurt more at night than during the day?

Daytime movement keeps the joint mobile and distributes load constantly; at night, the joint is still and, for side sleepers, compressed under body weight for hours at a time. Night pain in the shoulder is extremely common with arthritis and rotator cuff problems alike, and persistent night shoulder pain is always worth a GP conversation.
